Are your kids ready for school?
Dear Parent or Guardian,
Minnesota law requires children enrolled in child care, early childhood education, or school to be immunized against certain diseases, unless the child is exempt. Please take a moment to review your child's immunization records and ensure that they have received the necessary vaccines. If you have any questions or concerns about immunizations, please reach out to your school nurse or consult your healthcare provider.
Students entering 12th grade need to have documentation or a legal exemption for the booster dose of the meningococcal vaccine (Most children received the initial dose of this vaccine when they entered 7th grade).
Teenagers and young adults are at higher risk for meningococcal disease. This dose is important to make sure they are fully protected through their highest-risk years. Below is the Student Immunization form, included in it is exemption information as well.
