• Daily Home Health Assessment for Students and Staff
    Please complete the below health assessment daily before riding the school bus or arriving at school.
    If you have any questions regarding these symptoms, please contact your primary healthcare provider.


    If your student, or anyone in the household, is experiencing any of the below symptoms AND are awaiting results of a COVID-19 test - everyone in the household stays home until the results are received.


  • Do you have a "More Common" symptom:

    • Fever greater than or equal to 100.4 F?
    • New onset and/or worsening cough?
    • Difficulty breathing?
    • New loss of taste or smell
  • Or at least (2) of the "Less Common" symptoms:

    • Sore throat
    • Nausea
    • Vomiting
    • Diarrhea
    • Chills
    • Muscle pain
    • Excessive fatigue
    • New onset of severe headache
    • New onset of nasal congestion or runny nose
  • Have you had close contact with a person confirmed to have COVID-19?
    (close contact is within 6 feet of an infected persona for at least 15 minutes)

    Have you traveled to an area where the local or state health departments are reporting large numbers of COVID-19 cases?

  • Should I go to school?

    If answered YES to any question, stay home, and call your student's school building.  Staff should contact their supervisor.
