• Sports Physicals 

    • Students in Athletic Activities must have a sports physical on file in the activities office to participate.   

    • Sports physicals are good for three years from the date of the exam.

    • A well-child check does NOT qualify as a sports physical. 

    • A link to the 2023-2024 MSHSL Sports Physical Form has been posted to the Activities Website under the Shortcuts Category. Please print this form for your doctor to complete.

    • Once the physical form is completed by your doctor, please email a copy to Taylor Roles, DC Activities Secretary, at taylor.roles@dc.k12.mn.us and keep the original for your records.
