Welcome to Dassel-Cokato Health Services

About Us

  • The Health Services Program is staffed by a Licensed School Nurse and three Health Assistants who are certified in CPR and First Aid. As a team, we work together to meet the health needs of our students and promote wellness. The team has a unique understanding of students’ physical, social and emotional health needs and how wellness has an impact on learning.

Along with serving the day to day health needs of our students, we provide annual Hearing and Vision screening for grades K-3, 5, 7, 10 and for any students who are referred to us by a parent, teacher, or the students themselves. Scoliosis screening is also done for girls in grades 5 and 6. All screenings are provided as recommended by the Minnesota Department of Health. We provide referrals for further examination as needed, and can help connect families with care and resources.

Parents and Guardians

  • Please contact the Licensed School Nurse, Annette Bohnsack, if your child has special health needs, requires medication or nursing assistance during the school day, or if you have questions or concerns regarding your child's health.

Emergency Health Plans are developed for children with conditions like asthma, severe allergies, diabetes, seizure disorders, or other health condition that could become an emergency at school. Please notify the School Nurse if your child has one of these health conditions and we will work together to create a plan that can help keep your child safe and healthy at school.
